Registration, Cancellation, and Swap Forms
If you cannot access the online, real-time registration system because you are not an admitted student, these forms may be a convenient way for you to register for, cancel, add, or withdraw from classes. Use the "Register for classes" link to make grade-base or credit number changes to your registration.
These e-mail forms are not for printing. If you would rather fax or mail your registration, please print the PDF version of the registration cancel/add form.
Do not use these forms if you are registering for graduate level classes as a non-degree-seeking student and wish to receive graduate credit for those classes. Instead, complete a Request for Graduate Credit form.
